D 错是在后面那个 that，应该是指 patients，但却令读者以为是 emergency room。

E 错是因为 one nurse at least to be assigned for every four patients put through triage in a hospital emergency room 是个分句（one 前面省略了个 that），但是这个分句没有动词。还有就是 every four patients put through triage ina hospital emergency room 很别扭，虽然不是错，但是十分丑恶。
